Lost or Stolen Permit
You must file a police report for a lost or stolen permit and submit the incident report to NYC DOT. The incident report must state the permit type and permit number.

Permit Not Received
If you have been approved for a Parking Permit for People with Disabilities but never received it, you can report the problem by mail.
You can download the form online or request a copy by mail.
Please mail the completed form to the New York City Department of Transportation's Permits and Customer Service office.

Damaged Permits
To replace a damaged permit, the damaged permit must be returned to NYC DOT by mail or in-person (by appointment). A replacement will be issued once the damaged permit is received.
It may take up to 45 days to process a replacement for a permit mailed to NYC DOT office.
To replace a damaged permit, you must first fill out a Damaged Permit Form and describe the damage.
Return the completed form and damaged permit by mail or in-person (by appointment).
